This list may not include all of the insurance plans we accept.  Please check with your health plan provider listing to see if our provider is a participant with your insurance.

We are required by law to collect all  co-pays at the time of service. Our office participates with many insurance plans. Due to the constant change in coverage and benefits, we urge you to contact your insurance carrier to verify our participation. Please bring a current insurance card to every visit. If we participate with your insurance, we will be happy to submit the claim for you. We ask that you be prepared to pay any co-pay or co-insurance at the time of service.
